first what you need is a backpack. one with clip-straps that go around the chest and around the waist. These are needed so you can keep the back tight against you and it wont move around. Camel backs (hydration packs) work well because they are nice and small and designed to fit snug against you body. You can just take out the bladder and stuff your gear in there. The next thing you need is a pole. Something light weight but still strong, and something that wont bend easy or bounce around. You can always strengthen something up by attaching a splint to it, like a thin peice of wood or metal (duct tape is always your friend).
Next you need to attach the pole to the bag. You will need to build a base plate for you pole to attach. Remember you need to angle the pole upwards a little bit so it doesnt bash your head while you ride, otherwise try and get the pole to go out over one of your shoulders. The baseplate is something you can slip into the backpack and then insert the pole into. It should be as big as the bag's pocket so it doesnt move around while your riding, and if the pole is attached propperly it should be supported by the baseplate. This is the tricky part, trying to build a baseplate to securely attach your pole to that also fits inside the bag and keeps everything sturdy. If you need to tear a hole in the bag for the pole to stick through then so be it, as long as everything fits nice and snug your in business. Next you would want to get something to make your 'hook' out of (the part that the camera sits on). Depending on how heavy your camera is you will need something strong but bendable. Steve used part of a squash racket i have used part of an old tri-pod, but all u need to do is duct tape it to the end of you pole then bend it in a U shape so it points backwards to where the rider will be. Then attach your camera here.

Thats a pretty basic outline, finding the materials to make shit out of is the hard part, if you have a good imagination then you will make one easily...
like i said, the fun part is figuring out how to make it

its a do it yourself job. all you need is a small backpack (camel pack) and a pole that comes out of it, which has a kink in the end so the camera can point towards the rider. the length of the pole depends on the wideness of the camera, so its best to use something that can extend (tripod leg) so you can adjust to fit the rider in the frame.
